How to upload a 2567 to EasyPOC
A 90-second guide to making sure your CMS Form 2567 uploads cleanly the first time.
Why some 2567 PDFs won't upload
EasyPOC works by reading the text directly inside your 2567 PDF — every F-Tag, every finding, every regulatory reference. Most PDFs from CMS and state survey portals are text-based and upload perfectly. But some PDFs are actually images of the document rather than text — usually because someone opened the original and used “Print → Save as PDF” instead of downloading the file directly, or because the PDF is a scan of a paper copy. EasyPOC can't extract text from those, so it can't pull out the citations.
Download the original from your state portal
The fix is almost always the same: go back to where you got the 2567 from (state survey portal, CMS QIES system, or whatever email or system delivered it) and look for a direct “Download” or “Save” link rather than opening the file in a viewer and re-printing it. The file you want is the one your state generated — not a copy you made.
Tips by state
Quick guidance for the most common state portals. We're adding more as we encounter them.
California (CDPH)
California uses the CDPH HFID portal. Look for the “Download PDF” link rather than the “View” or “Print” option.
Texas (HHSC)
Texas LTC providers receive 2567s through TULIP. Save the file directly from the email attachment or the TULIP download link — don't print and re-save.
Florida (AHCA)
AHCA delivers reports through the FHFIS portal. Use the “Save As” function in your browser to download the original.
New York (NYSDOH)
NYSDOH delivers 2567s via the HCS portal. Right-click the PDF link and select “Save link as...” to download the original.
Oklahoma (OSDH)
Look for any “Download” link in the survey notification email or OSDH portal — avoid using your browser's Print function.
Ohio (ODH)
Look for any “Download” link in the survey notification email or ODH portal — avoid using your browser's Print function.
Pennsylvania (DOH)
Look for any “Download” link in the survey notification email or DOH portal — avoid using your browser's Print function.
Quick check: is your PDF the right type?
If you open your 2567 PDF in any viewer and try to highlight or copy text, you should be able to select individual words and paragraphs. If clicking and dragging just selects rectangular regions or nothing at all, the PDF is an image and won't upload to EasyPOC. The fix is to go back to your state portal and download the original.
What if I only have a printed or scanned copy?
It happens — sometimes the only version of a 2567 a facility has on hand is a scan or a printout. In those cases, email the file to support@easypocapp.com with a quick note about your facility and survey date, and we'll process it manually for you. Turnaround is usually within 24 hours.
